Select all Fuel Type(s) for this Report: Choose the fuel type(s) you want to file in this return by checking the box next to the fuel type you are reporting, and select Continue
Select No Operation: If you did not operate a qualified motor vehicle(s) check the box and select Continue.
Amended Return
If there are no changes, choose Continue.
If you need to add or change the fuel type, check the box next to the fuel type and select Continue.
If you deselect a fuel type, all previously reported data will be lost.
Select: No Operation: If you did not operate a qualified motor vehicle(s), check the box and select Continue.
