Purpose: Determine if a taxpayer is responsible for paying the 911 Assessment.

Do you collect any of the 911 emergency communication fees under Health & Safety code, Chapter 771? Select Yes or No.

If yes, enter the first business date for each THAT APPLY. Enter the date of taxable sales for Wireless Emergency Fee, Emergency Service Fee or Equalization Surcharge.
