Entity AddressSpecial Purpose District Reporting

Mailing Address: Enter the address where the special purpose district receives correspondence.

If the entity's mailing address and physical address are not the same, select No. You will be asked to provide the physical address on the next screen.

Note: the form uses address-matching software that evaluates accuracy. If the address entered does not match USPS records, it will ask to confirm the address by checking the box.
