Texas Education Freedom Accounts Program Integrity Reporting Form

YOUR REPORT IS PUBLIC

Information reported to the Texas Education Freedom Accounts (TEFA) program is open to the public under Texas law. Any member of the public, including media representatives, may file an open records request and obtain a copy of your report and any attachments. DO NOT INCLUDE sensitive, confidential or personally identifying information that could be used to uniquely identify you or your family, either directly or through contextual combination such as name, address, phone number, email address, race, ethnicity, religion, date of birth, Social Security number, government-issued ID, taxpayer ID, unique TEFA program identifiers, professional/occupational license numbers, school/employer name or location, unique digital/electronic ID, banking/financial account numbers, medical/health information or vehicle information.

  • Pursuant to Texas Government Code Section 552.101, information considered confidential by law—including birth certificates, dates of birth, and documentation establishing lawful admission—is protected under the doctrine of common-law privacy and is excepted from public disclosure.
  • Confidential information and personally identifiable student information collected and maintained by the agency or its Certified Educational Assistance Organization pursuant to Texas Education Code Chapter 29, Subchapter J, is excepted from public disclosure.
  • Pursuant to Texas Government Code Section 552.114, education records — including individualized education programs, disability verification documents, and personally identifiable student information — are confidential and excepted from public disclosure.
  • Pursuant to Texas Government Code Section 552.114 and 552.101, special-education eligibility information, disability status records, and Texas unique student identifiers contained in education records are confidential and excepted from public disclosure.
  • Pursuant to Texas Government Code Section 552.117, Social Security numbers, Individual Taxpayer Identification Numbers, and any information that relates to a person's home address, home telephone number, emergency contact information, or family member status are confidential and excepted from required public disclosure.
  • Pursuant to Texas Government Code Section 552.101, personal financial information—including federal income tax returns, adjusted gross income, wage data, and other intimate financial details—is confidential under common-law privacy and is excepted from public disclosure.
  • Pursuant to Texas Government Code Section 552.101 and 552.117, driver's license numbers, state identification numbers, and other residency-verification records containing personal identifying information are confidential and excepted from public disclosure.
  • Pursuant to Texas Government Code Section 552.117 and 552.024, emergency contact information and information revealing family relationships are confidential and may not be released in response to a public information request.

The TEFA program takes seriously all reports of fraud, theft, waste and abuse. All reports submitted through this form will be reviewed by the TEFA program, and the Comptroller's office will investigate allegations of illegal acts and other improprieties. Anyone can report suspected improprieties by completing this report.

Is your report related to the TEFA program?

  • If you suspect fraud, theft, waste, abuse or scams involving the Comptroller's office that are not related to the TEFA program, including attempts to impersonate the Comptroller's office to perpetrate illegal acts on the public, refer to the agency's General Fraud Reporting webpage for more information.
  • If you suspect tax avoidance or falsified records related to the Comptroller's office that are not related to the TEFA program, report it to the agency's Criminal Investigations Division.

How to submit a TEFA-related report

To submit a TEFA-related report, please complete the online form below as thoroughly as possible. A report is not submitted until you select the “Submit Report” button and a confirmation message appears.

Submitted information will be assessed for appropriate action. The Comptroller’s office does not provide status updates for investigations.

Information submitted to the Comptroller's office, including name and contact details, may be released pursuant to a Public Information Act request or court order. For details on how the agency collects, uses, stores and discloses information you provide, refer to the agency’s Privacy and Security Policy. Reports also may be disclosed to the State Auditor's Office (SAO), the Attorney General's office and/or investigatory or law enforcement agencies at the Comptroller's discretion.

The SAO also investigates allegations of misuse and/or illegal acts involving state resources. Reports may be submitted to SAO’s Fraud Hotline by phone at 800-TX-AUDIT (892-8348) or through the State Auditor’s fraud website.

Protecting your identity when submitting a fraud report can reduce the risk of retaliation and help ensure your information is taken seriously. Consider the following precautions before submitting a tip:

  1. Use a VPN

    Connect through a reputable Virtual Private Network (VPN) before accessing the reporting portal or sending any communication. This masks your IP address and general location, making it harder to trace the report back to your device or network.

  2. Strip metadata and geolocation from files and photos

    Documents, images, and PDFs often contain hidden metadata — such as author names, device information, timestamps, and GPS coordinates — that can inadvertently identify you.

    Before submitting any attachments:

    • Remove EXIF data from photos (many phones embed location data automatically).
    • Check document properties (Word, PDF, Excel) and clear "Author," "Company," and "Last Modified By" fields.
    • Consider using a metadata-removal tool or taking a screenshot of the file instead of submitting the original, if authenticity isn't compromised by doing so.
  3. Create a single-use email address

    Set up a dedicated, anonymous email account solely for this report — one not linked to your name, phone number, or other personal accounts. Avoid reusing passwords or recovery information tied to your identity and access this account only through the VPN connection mentioned above.

    Additional considerations:

    • Avoid submitting the report from a work or personal device that could be tied to your identity through account logins or network records.
    • Do not discuss the report on channels (email, chat, social media) linked to your real identity.

Description of Suspected Fraud, Illegal Act or Other Impropriety

Describe the alleged fraud, theft, waste, abuse, illegal act or other misconduct and your knowledge of those activities. Please provide as much detail as possible. Specify who, what, when, where, how and why — or as much as you know. Include information for all people and entities involved — including other potential witnesses. This can include names, contact information and other relevant known information. (Required)

    Disclosure Statement: Providing a Social Security number is voluntary. If known, an SSN may be included for fraud reporting purposes. Information on this form may be released under the Texas Public Information Act and applicable federal law.

    State Privacy Rights Notice: With few exceptions, you are entitled to request the personal information the Comptroller's office collects about you. Under Texas Government Code Sections 552.023, 559.003 and 559.004, you are entitled to receive and review personal data and the Comptroller's office must correct inaccurate information.
