A location must be active for sales tax in order to add a permit type to your renewal application.

Select Yes from the dropdown to add the permit type to the location.

Location Information

The location must be a commercial business location. If any of the information is incorrect, please contact us.

First Business Date

This date cannot be more than 90 days in the future or prior to the sales tax start date. If your start date is prior to the current date you must contact us.

Please allow 4 to 6 weeks for processing new permit types.
