Supporting Documentation

Submit the following forms with your Distributor Monthly Report of Cigarettes and Stamps:

• Form 69-108, Texas Distributor Receiving Record of Cigarettes
• Form 69-109, Texas Distributor Receiving Record of Cigarette Tax Stamps
• Form 69-315, Texas Certificate of Tax Exempt Sale (required if you report tax exempt sales for the filing period)

Supported file formats:

.jpeg, .jpg, .tff, .gif, .png

Supported file size:

File size must be less than 5 MB per file. Multiple files are allowed

File:

Navigate to the file, select it, and choose Open to attach the file.

When you have uploaded your files, select Continue.
